Frequently asked questions
Is an affidavit enough to correct my name?
It supports the request. The issuing office decides, often also asking for the original document and proof of the correct name.
Do I need a newspaper notice for a spelling correction?
Usually not for a simple spelling error, but some offices ask for one. A full change of name generally needs newspaper and Gazette publication.
Is this the same as a name change affidavit?
No. A correction says the record is wrong; a change says you now use a different name.
